Sex-Specific Clinical Outcomes After Treatment of Left Main Coronary Artery Disease. A NOBLE Substudy

BackgroundWhile female sex has been associated with worse outcomes following coronary revascularization, previous analyses in left main artery (LMCA) disease have conflicting. In addition, a signal that increased mortality may be specific to women treated percutaneous intervention (PCI) requires further investigation.MethodsNordic-Baltic-British revascularization study (NOBLE) was randomized trial comparing PCI bypass surgery (CABG) patients LMCA disease. The primary endpoint composite of all-cause mortality, nonprocedural myocardial infarction, repeat and stroke (major adverse cardiovascular cerebrovascular events [MACCE]). We report the 5-year sex-specific outcomes.ResultsOf 1184 analyzed, 256 (22%) were 928 (78%) male. There no significant within-sex differences baseline characteristics, location, or complexity between those CABG. MACCE rates 29% 15% females 28% 20% males CABG, respectively. Within both sexes, there an risk compared but difference mortality. On multivariate analysis, not independent predictor MACCE.ConclusionsFollowing treatment disease, long-term favored CABG over sexes. Importantly, at 5 ​years.
